Medical Home Port Teams
The Medical Home Port process is a team based approach to primary health care where patients are assigned to a specific Medical Home Port Team. The team reviews all of the patient’s medical needs and ensures that anything required for the patient is addressed during the appointment including booking referrals and missing immunizations. The Medical Home Port process is an integrated approach to delivering health care to the patient, and it provides a wider scope of services in clinics for the patient.
Along with doctors, nurses and corpsmen, the Medical Home Port Team approach involves other specialists within each clinic such as a pharmacist who can dispense some medications, a case manager, diabetic specialist and a behavioral health specialist to help with chronic habits such as insomnia, smoking or weight loss.

On federal holidays, all Medical Home Port Teams are closed, but the Nurse Advise Line is open 24/7/365 at 1-800-TRICARE (874-2273), OPTION 1. You can also use TRICARE’s Online Patient Portal to make appointments, refill prescriptions, send secure messages to your team or access medical records on the
MHS Genesis Portal.
